Approved applicants will receive a secure payment link to complete their subscription setup. No payment information is collected at this stage.

Self-Pay membership is $30 per month. Your card will not be charged until your application is reviewed and approved. Upon approval, you will receive a secure link to complete payment.

The aSuggestion team can also assist individuals and families with Waiver Funding to help offset subscription fees. Mention this in your care team notes if applicable.
